Background: After a caesarean delivery, efficient pain management enhances a woman's capacity to function & engage with her newborn. Aim: to evaluate the impact of local wound infiltration on studied cases undergoing caesarean sections' pain levels & need for postoperative analgesia. Patients & methods: This had been a prospective randomized comparative controlled clinical trial conducted on 96 full-term pregnant women undergoing elective cesarean section at the Obstetrics and Gynecology Department, Kasr El-Aini Teaching Hospitals, Cairo, Egypt. The study lasted six months, from November 2021 to May 2022. Results: The study group expressed lower pain scores during the subsequent readings (p < 0.001) compared to controls, indicating proper pain management in the wound infiltration group. There was a significant increase in nausea and vomiting in the control group, with 20.8% and 41.7% of cases reporting these symptoms. However, bradycardia and hypotension did not show a significant difference. Additionally, 14.6% and 37.5% of women reported pruritis, shivering, and wound dressing changes. The level of patient satisfaction was significantly better in the study group (p< 0.001) when compared to controls. Conclusion: wound infiltration with local anesthetics appears to be an effective method for pain relief after cesarean section. It is associated with lower opioid consumption, prolongation of the time to the 1st rescue analgesic, a decrease in post operative pain-related complications, and better patient satisfaction after the procedure.
