Diabetic neuropathy, a common complication of diabetes mellitus, manifests as progressive nerve damage affecting various organ systems, notably leading to diabetic foot ulcers (DFUs). This review synthesizes current knowledge on the pathophysiology, risk factors, and classifications of DFUs. It highlights the significant impact of hyperglycemia, hypertension, dyslipidemia, age, glycemic variability, and genetic factors on neuropathy development in diabetes patients. Various classification systems, including Wagner, PEDIS, University of Texas, and S(AD)SAD, are discussed for their utility in assessing DFU severity and guiding treatment decisions. The COVID-19 pandemic worsened DFU management globally, increasing amputation rates in many countries. The management strategies like glycemic control, drug therapy, vascularization enhancement, exercise, and nutritional interventions. Advanced therapies such as hyperbaric oxygen therapy, electrical stimulation, negative pressure wound therapy, and bioengineered skin show promise in treating severe DFUs. This comprehensive review underscores the multidimensional approach required for effective DFU management, aiming to mitigate complications, reduce amputations, and improve patient outcomes amidst evolving clinical landscapes.
