Background: Previous research has explored the association between Helicobacter pylori (H.pylori) infection and Gastric Cancer(GC). Objectives: The aim of this study was to investigate the impact of H.pylori status on the clinicopathological features and prognosis of gastric cancer patients in El-Bayadh region. Methods: Totally 126 patients diagnosed with gastric carcinoma at the Oncology Department of the EPH "Mohamed Boudiaf" in El-Bayadh between January 2015 and December 2022 were retrospectively analyzed. The relationship between H.pylori infection status, clinicopathological parameters, and survival time of GC was examined. Results: This study included 126 cases, of which 100 (79.36%) were H. pylori-positive, while 26 (20.64%) were negative. H.pylori infection was more prevalent in men (54 cases)(p=0.03) and older individuals (82 cases). Gastric carcinoma with H.pylori-positive status showed predominantly intestinal and diffuse adenocarcinoma (p=0.002). A statistically significant correlation emerged between H.pylori positive patients and lymph node metastasis (p=0.01) and HER2 overexpression (p=0.05). Overall survival (OS) correlated with H.pylori infection status, with H.pylori-negative patients tending to survive longer than positive ones (p= 0.03) Conclusion: Our study suggests that H.pylori infection is a major risk factor for gastric cancer in our population, but there are also other factors involved in the development and progression of this disease.
