Alendronate-Based Dentin Surface Treatment: A Narrative Review on Bonding Performance of Dental Adhesives

Abstract

The long-term success of adhesive restorations is largely dependent on the stability of the resin–dentin bond, which tends to deteriorate over time due to enzymatic degradation of the exposed collagen matrix. Matrix metalloproteinases (MMPs) and cysteine cathepsins are key contributors to this degradation, particularly under acidic conditions typically associated with adhesive procedures. This narrative review explores the potential of sodium alendronate, a potent MMP inhibitor, as a dentin pre-bonding treatment aimed at enhancing the performance and durability of adhesive systems. This review suggests that alendronate pre-binding treatment improves both immediate and aged bond strength, reduces nanoleakage, and stabilizes the hybrid layer by inhibiting collagenolytic activity. These beneficial effects have been reported across various adhesive strategies, including both etch-and-rinse and self-etch protocols. Consequently, alendronate-based pre-bonding treatment appears to be a promising approach for improving the longevity of dentin bonding. However, further clinical investigations are needed to confirm its long-term effectiveness and safety in restorative dentistry.
