OBJECTIVE The BFP (buccal fat pad) is a unique anatomic structure that is distinct from the other fat compartments of the face, earlier only used for oro-antral fistulas closure has undergone a significant transformation in its perception within the field of reconstructive oral and maxillofacial surgery owing to its versatility, high success rates and minimal complications. This study evaluates the effectiveness of buccal pad of fat in the reconstruction of a wide range of maxillofacial defects caused by traumatic injuries, oncologic resections, benign tumors, cysts, congenital malformations, reconstruction of the TMJ (Temporomandibular Joint), coverage after fibrotomy in oral submucous fibrosis, and augmentation of tissue volume in a host of other reconstructive procedures. METHODOLOGY A retrospective analysis was done for patient 13, who had undergone reconstruction with the buccal pad of fat for varied maxillofacial defects. RESULTS Successful epithelialization of the flap was seen within a period of four to six weeks consistently in all cases. There was no soft tissue contraction or functional impairment seen in any case in the follow-up period. Complete closure of the defect was achieved in 11 patients. CONCLUSION Several advantages of the buccal fat pad exist over other types of flaps. The surgical technique is simple and has been shown to have a high success rate in a multitude of uses. Thus, because of its versatility, ease of harvesting, and promising results, with a relative lack of complications, the authors highly recommend the use of the pedicled buccal pad of fat for the reconstruction of maxillofacial defects.
