Understanding both normal and aberrant root canal anatomy is crucial for successful endodontic treatment. Clinicians must be aware of the incidence of various anatomical variations in mandibular molars and conduct thorough preoperative radiological assessments from multiple angles. Proper access preparation and comprehensive examination of the pulp chamber are essential for locating and debriding all canals, thereby ensuring successful endodontic therapy. This report discusses the variations associated with the middle mesial canal (MMC) in mandibular molars.
