Introduction: Periodontal abscess is a localized purulent infection of the periodontal tissues and mainly caused by untreated periodontitis and the presence of foreign objects around the teeth. When left untreated, the prognosis of tooth involved periodontal abscess is worsen and can lead to tooth loss. Treatment may involve non-surgical and surgical periodontal therapy. The purpose of this article is to describe the combination of bone graft and host modulation therapy for treating periodontal abscess Case Report: A 46-year-old male patient came to Periodontology Specialist Poly of Dental and Oral Hospital Airlangga University with chief complain that the gums around lower left back teeth are swollen, pain when chewing food, and have been discharging pus for the past 2 weeks. The patient didn’t have a history of systemic diseases and allergy Case Management: Treatment for this case including scaling and root planing, antibiotic prescription, abscess incision, and surgical procedure of open flap debridement with bone graft. After surgical treatment, patient also given host modulation therapy and carried out for up to 6 months showing good results Conclusion: Combination of bone graft and host modulation therapy show the good results and may considered as a treatment option for treating periodontal abscess
