Platelet-rich fibrin (PRF) has emerged as a promising autologous biomaterial in bone regeneration for maxillofacial surgery. This randomized controlled trial evaluates the effectiveness of PRF in enhancing bone healing in patients undergoing maxillofacial reconstructive procedures. A total of 80 patients were randomly assigned into two groups: the PRF group, where PRF was used in conjunction with autogenous bone grafts, and the control group, which received standard bone grafting alone. Radiographic and histological assessments were conducted at 3 and 6 months postoperatively to evaluate bone density, trabecular pattern, and graft resorption rates. The results demonstrated statistically significant improvement in bone density in the PRF group (p<0.05), with accelerated osteogenesis and reduced graft resorption compared to the control. Histomorphometric analysis revealed increased new bone formation in PRF-treated sites (p=0.02). The findings suggest that PRF enhances osteoconductivity and osteoinductivity, promoting faster and more predictable bone regeneration in maxillofacial surgeries. This study highlights the clinical relevance of PRF as an adjunctive material in bone grafting procedures, addressing critical gaps in regenerative oral and maxillofacial surgery.
