Background: Varicose veins (VV) are a common venous disorder characterized by dilated, tortuous veins, primarily affecting the great saphenous veins (GSV) and small saphenous veins (SSV). Traditional surgical treatments have been associated with high recurrence rates and significant complications. Endovenous laser ablation (EVLA) has emerged as a less invasive alternative with promising outcomes. Objectives: This study aimed to evaluate the efficacy of a low-profile endovenous laser catheter in managing all sites of reflux in lower limb primary varicose veins. Methods: A prospective study was conducted on 50 patients with GSV incompetence and reflux at the SFJ, confirmed by duplex ultrasound, requiring surgery. Patients underwent comprehensive preoperative evaluations, including history taking, clinical examination, and sonography. Postoperative follow-up included Doppler studies and monitoring for complications at 3, 6, and 18 months. Results: The mean age of the study cohort was 40.66±9.45 years, with 54% male and 46% female patients. CEAP classification revealed 54% were C3, 22% C4, 14% C5, and 10% C6. Post-treatment, 100% of patients showed improvement with no residual varicosities. Complications included neuropathy (26%), pigmentation (10%), ecchymosis (4%), and burns (2%). Thrombophlebitis occurred in 10% of cases, particularly associated with the presence of perforators (p=0.006) and SSV involvement (p=0.048).
