Evaluation of Anterior Sector Drainage in Rt Lobe Living Donation Using Autologous Interposition Grafts

Abstract

Objective: to evaluate the efficacy of using natural grafts as interposition grafts for drainage the anterior sector of the liver during rt lobe donation for adults Material and Methods: This retrospective randomized study included 39 patients where anterior sector was drained using natural grafts in our experience for rt lobe liver transplantation at Gastro intestinal Surgical Center, Mansoura University from 2004 till 2021. Results: The patency rate of the natural conduits was (92.32%) and 5 cases show graft congestion in the anterior sector (12.81%), ascites was reported in 2 cases (5.1%), no cases reported SFSS. during the 2nd week, only 3 cases (7.72%) show graft congestion. serum albumin show improvement with time from 3.622±0.431 gm/l in the 1st week to 3.812 ±0.51 gm/L in the 1st month bile leak occurred in 3 cases (7.72%), ERCP was done for only one cases (2.61%) and the other 2 cases were managed conservatively. Biliary stricture was reported in 2 cases at the 1st month managed by ERCP. Univariate analysis and Multivariate analysis in our study reported that there was no predictive value for the conduit patncy (GRWR p=0.099, operative time p=0.716 Conclusion: The natural conduit as interpositional graft for drainage of MHV tributaries was effective with high patency rate in the 1st post-operative week (92.3%) with good post-operative outcomes as regard liver function tests and clinical finding as ascites and portal hypertension improvement with acceptable rate of biliary complication. but the main barrier was the long WI time.
