Melanoma and dysplastic nevi share overlapping histopathological features, posing diagnostic challenges with significant clinical implications. This case-control study aimed to elucidate distinguishing histopathological markers of melanoma versus dysplastic nevi through a comprehensive analysis of tissue samples from 120 patients, stratified equally into melanoma and dysplastic nevi groups. Comparative analyses revealed statistically significant differences in nuclear atypia, mitotic index, and architectural disarray (p < 0.001), alongside novel markers, such as stromal changes and immune cell infiltration patterns, identified as potential diagnostic tools. Results highlight an innovative diagnostic criterion that improves differentiation accuracy. Our findings underscore the relevance of incorporating these markers into routine histopathological evaluation to enhance diagnostic precision. This study bridges existing gaps by identifying previously underexplored markers and establishing their statistical relevance, offering a significant contribution to melanoma diagnostics.
