The purpose of this research is to examine how the ISBAR3 protocol implementation affects the effectiveness of patient handover and nursing documentation at Pariaman Regional Public Hospital. The studies used a qualitative approach through library research, involving systematic analysis of literature related to ISBAR3 implementation in healthcare settings during 2020-2024. Results showed significant improvements in documentation accuracy (78%), reduction in patient handover duration (45%), and decrease in patient safety incidents (47%). Protocol implementation resulted in increased staff satisfaction from 65% to 89%, with response time to patient deterioration reduced from 45 minutes to 15 minutes. The protocol adaptation process considered local characteristics and resource limitations, producing an effective and sustainable implementation model for regional hospital contexts.
