Multilayered closure technique for surgical repair of a secondary symptomatic oronasal fistula in non-cleft patients

Abstract

Objectives: the aim of our study was treatment of oronasal fistula in non-cleft patients by using single layer technique (only mucosal flap) and multilayered technique (nasoseptal flap nasally, Platelet-rich fibrin, buccal pad of fat and mucosal flap orally) and comparing the results between the two techniques Methods: This clinical, interventional, prospective, and randomized controlled study was carried out on individual chosen from the outpatient clinic. 20 patients (12 males and 8 females) required closure of non-cleft oronasal fistula. Patients had been divided into two equal groups, Group I (control group): closure of the defect with single layer technique. Group II (study group): closure of the defect with multilayered technique. Results: Study group showed statistically significantly higher pain score than control group (P-value = 0.0073). Study group showed statistically significantly less edema than control group (P-value = 0.001). Study group showed statistically significant higher success rate and complete closure without any recurrence more than control group (P-value < 0.056). Surgery time was longer in study group than control group. Study group showed more post operative complication such as epistaxis than control group Conclusion: Oronasal fistula (ONF) is an aberrant epithelized pathway connecting the mouth and nasal cavities, it may be repaired by simple local flaps (single layer technique) but the recurrence rate is very high compared with the multilayered technique which is arranged as follows, form the nasal aspect we had nasoseptal flap (Hadad Bassagasteguy). From the oral aspect, it’s arranged from bottom to top as follows, Platelet-rich fibrin layer, buccal pad of fat layer, and mucosal layer. Although it is more time-consuming and needs high technical skills, it has a very high success rate with no recurrence according to our study.
