OUTCOME OF MODIFIED MILLARD TECHNIQUE USING CONCEPT OF FUNCTIONAL ZONE REPAIR IN UNILATERAL CLEFT LIP REPAIRED PATIENTS

Abstract

BACKGROUND: Cleft lip is a frequently occurring congenital condition with a multifactorial origin. Unilateral cleft lip is approximately nine times more prevalent than its bilateral counterpart and occurs twice as often on the left side compared to the right. The surgical correction of cleft lip is considered one of the most significant procedures in plastic surgery. OBJECTIVE: To determine the postoperative outcomes of modified Millard technique using concept of functional zone repair in unilateral cleft lip repaired patients METHODOLOGY: A prospective observational study was conducted at Department of Plastic Surgery. Non probability convenience sampling was used to include 73 patients of both gender. The Postoperative Outcomes were assessed during follow-up visits at 1 month, 3 months, and 6 months postoperatively and included aesthetic Evaluation, Functional Assessment and postoperative complications. RESULTS: The most affected age group was 6 months to 2 years (45.2%). Left unilateral cleft lip was more common (64.4%) than right unilateral cleft lip (35.6%). Postoperatively, aesthetic and functional outcomes were reported very favorable. Complications included vermilion notching (15.1%) and nasal deformities (8.2%). Younger patients and males exhibited significantly better results in both aesthetic and functional aspects (p = 0.032). CONCLUSION: The Modified Millard technique provides excellent aesthetic and functional outcomes, particularly when performed at an earlier age. Male patients showed better overall results compared to females.
