The relationship between admission blood pressure and mortality in stroke patients

Abstract

Background: Blood pressure (BP) at admission can be a critical parameter for managing stroke patients. The relationship between BP with mortality is a significant area of clinical research. This study aimed to investigate the relationship between admission BP and mortality in stroke patients. Methods: This cross-sectional study was conducted on 123 stroke patients aged over 40 years. Patients with a clinical diagnosis of acute stroke were included in this study. The data were collected for demographic characteristics, medical history, and admission BP measurements. The relation between mortality rates were evaluated with systolic BP (SBP), diastolic BP (DBP), mean arterial pressure (MAP), and various patient characteristics. Results: Higher admissions systolic, diastolic and mean of blood pressure had significant relations with increased mortality rates in stroke patients. Patients with SBP ≥ 140 mmHg had an 18.9% mortality rate compared to 2.9% in patients with SBP < 140 mmHg (OR: 7.9, CI: 1.6–37.8, P = 0.004). Similarly, patients with DBP ≥ 90 mmHg had a mortality rate of 23.3% compared to 2.5% in those with DBP < 90 mmHg (OR: 11.8, CI: 2.4–56.9, P < 0.001). Mortality was also higher among patients hospitalized for 7 days or more (17.3%) compared to those with shorter stays (4.5%, OR: 4.3, CI: 1.1–17.1, P = 0.031). Other factors, such as age, gender, and comorbidities did not show associations with mortality. Conclusion: In conclusion, the results show the importance of BP management at admission as a predictive factor in stroke patients.
