The Role of Glycemic Control in Modulating the Progression of Diabetic Nephropathy: New Insights from Recent Clinical Studies

Abstract

Diabetic nephropathy (DN) is one of the most frequent and serious complications of diabetes with a progressive reduction in renal functional reserve. In this study, the progress of diabetic nephropathy was examined through the impact of glycemic control on its course with the emphasis mainly on recent clinical data. The aim is to describe the association between hemoglobin A1c (HbA1c) levels and patients’ renal outcomes in diabetes. In the past few years, the evidence also showed that appropriate glycemic control reduces the risk for progression of DN, demonstrating a statistically significant reduction of albuminuria and improvement of GFR throughout the follow-up period. The statistical analysis points out a strong inverse relationship between HbA1c levels and renal functions with p value of less than 0.001. This implies that patients who manage their glucose level over time not only reduce DN progression but are even offered possibilities to protect the kidneys counter to beliefs whereby focus was only on agents that protect the kidneys. As shown by the evidence renal function deterioration in diabetic patients can be effectively averted by ensuring early and aggressive interventions to gain control of blood sugar levels. It carries important practical implications for clinicians for the proper control of diabetes which could potentially improve renal outcomes in the long-term.
